Financial Information: We believe it is important to not only provide the highest quality endodontic care possible, but to also make this care affordable as well. We have made arrangements to help make your treatment both convenient and flexible. Please ask us any questions you may have, as we are glad to be of assistance. Payment in full is required at the time of treatment with cash, check or credit card. For your convenience, we accept MasterCard, VISA and Discover. A note to our patients with insurance: As a service to you, we are happy to process any insurance claim, explain how your plan works and how to maximize your benefits, help with insurance estimates, and provide any additional documentation requested. Please keep in mind that we can only provide an estimate of your benefits and that you are responsible for all fees in their entirety. We do ask that you understand that your insurance plan is a contract between your employer and the insurance company. The benefits you receive and the type of plan chosen by your employer determines the fees “allowed.” We are proud that our fees reflect the individualized time the doctor and staff spends with the patient as well as the overall quality of the care and service provided. Stevens Practice Limited to Endodontics 2365 Park Street Jacksonville , FL 32204 (904) 388-1220 Fax: (904) 388-8220 E-Mail: BHSendo@aol.com Dr. Stevens earned his undergraduate degree in biology at Tulane University in 1984 and his dental degree at NYU College of Dentistry in 1988. He completed a two year general practice residency at North Shore University Hospital / Cornell University Medical College in Manhasset NY, and earned his Endodontic certification at The Albert Einstein Medical Center in Philadelphia, in 1992. He maintains a private endodontic practice in Jacksonville. Dr. Stevens is a Past President of the Northeast District Dental Association and the Jacksonville Dental Society. He has served on the Public Protection Committee, the delegation of the Florida Dental Association and the American Dental Association, the board of directors for the Florida Dental Health Foundation, FLADPAC and DENTPAC. Dr. Stevens is a Fellow of the American College of Dentists, The International College of Dentists, a member of The Pierre Fauchard Honorary Dental Academy, The American Association of Endodontists, and The Florida Association of Endodontists. Dr. Stevens and his wife, Rori, enjoy traveling and wine tasting. They have two children, Josh and Carly.
